IN CONSIDERATION OF BEING PERMITTED TO PARTICIPATE IN ANY WAY, BY SIGNING UP FOR A SUBSCRIPTION YOU AGREE TO THE FOLLOWING:
Acknowledge that I am familiar with the practice of Martial Arts and understand the rules governing these activities and the importance of these rules. Understand that the practice of Martial Arts involve various activities including, but not limited to, weapons work, striking, kicking, punching, hitting, throwing, joint manipulation including compression and locking, falling, choking, and the activation of pressure points and nerve attacks.
Agree that prior to participating, I have the right and responsibility to inspect the mats, equipment and facilities to be used, and if I believe that anything is unsafe, or beyond my ability, I will immediately advise my instructor of such conditions and refuse to participate.
Acknowledge and fully understand that I will be engaging in a contact activity that might result in serious injury, including permanent disability or death and severe social and economic losses due to my own actions, inactions, or negligence, but also to the actions, inactions or negligence of others, the rules of practice of Martial Arts, or condition of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to me or not reasonably foreseeable at this time.
Knowing the risks involved in the practice of Martial Arts o, I assume risk and accept personal responsibility for the damages following such injury, permanent disability or death.
I hereby release, waiver and discharge and covenant not to sue Modern Arnis Academy or any affiliates, their respective administrator, instructors, coaches, guest instructors, and other employees or volunteers of the organization, event officials, medical personnel, advertisers, other employees or volunteers of the organization, event officials, medical personnel, advertisers, other participants, their coaches, sponsoring agencies, sponsors and if applicable, owners, lessors, and lessees of the premises used to conduct the event, all of whom are hereinafter referred to as the releasee, from any and all claims, demands, losses, or damages on account o injury and death or damage to property, caused or alleged to be caused in whole or in party of the negligence of the releasee or otherwise.I HAVE READ THE ABOVE WARNING, WAIVER AND RELEASE AND UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SUBSCRIBING TO THIS WEBSITE, AND KNOWING THIS, VOLUNTARILY SUBSCRIBE. I AGREE TO PARTICIPATE KNOWING THE RISK AND CONDITIONS INVOLVED AND DO SO ENTIRELY OF MY OWN FREE WILL. I FURTHER AGREE TO FOLLOW ALL RULES, GUIDELINES AND INSTRUCTIONS GIVEN TO ME. I ALSO DECLARE THE PERSONAL INFORMATION I PROVIDE IN REGISTRATION IS TRUE, CORRECT, AND COMPLETE.
B. Cancellations
Cancellations may be done by you, the user, anytime by logging into your PayPal account and following the proper steps to cancel your subscription. There are no refunds once a subscription has been made, renewed, or cancelled. One a user cancels a subscription, there will be no further charges.
The administrators of this website have the authority to cancel your membership for any of, but not limited to, the following reasons:

No refunds will be issued after monthly subscription has been charged. It is up to the responsibility of the user to cancel their subscription before the renewal date. Subscription can be cancelled at www.paypal.com
